This Consumer Health Data Notice applies to consumer health data, if any, collected through the AFH Resource Connect LLC website.
AFH Resource Connect LLC is an independent directory, networking, advertising, and business resource platform serving the Adult Family Home industry in Washington State. We are not a referral agency, placement agency, or care coordination service.
We do not intentionally request or require consumer health data through our general website forms. However, users may voluntarily provide care-related or health-related information in messages or inquiries. If such information is submitted, it may be considered consumer health data under Washington law and will be handled in accordance with this Notice. Washington’s law applies broadly beyond traditional healthcare providers and requires specific policy disclosures.
If a user voluntarily submits it, we may collect care-related or support-needs information, information showing interest in long-term care, housing, or care-related services, communications that reference health conditions, mobility needs, supervision needs, or care preferences, and contact information submitted together with a care-related inquiry.
If consumer health data is voluntarily submitted, we may use it only as reasonably necessary to respond to inquiries and messages, communicate with users regarding their requests, process, display, or route inquiries through website functions where applicable, and operate, maintain, secure, and improve the website and its communications tools.
We may receive consumer health data from users directly through website forms, contact submissions, emails, and messages, directory inquiries or listing-related submissions, and other communications voluntarily sent through the website.
If voluntarily submitted, we may share consumer health data only to the extent reasonably necessary to respond to a user request or operate the website, including inquiry details submitted by the user, contact information submitted with the inquiry, and care-related information voluntarily provided by the user.
Where applicable, we may share consumer health data with hosting providers, website form or communications service providers, email service providers, analytics or website support providers, and listed providers, vendors, consultants, or professionals relevant to the user’s inquiry.
We do not sell consumer health data.
Washington residents may have rights, subject to applicable law and verification requirements, to confirm whether we collect, share, or sell consumer health data and access that data, withdraw consent where applicable, request deletion of consumer health data, and appeal a refusal to take action on a request.
